Congratulations on the birth of your baby!
You must register the birth of your baby at your local Council, who will issue you with a Birth Certificate. Afterwards, please register your baby at our Practice by following the steps below.
Please do not omit any of the STEPS.
STEP 1: Complete and submit the New Patient Registration form
*NHS No – It can be found on the front page of The personal child health record (Red Book).
STEP 2: Email us a copy of the Birth Certificate

IMPORTANT: The Birth Certificate is necessary to properly document the parents/guardians of the newborn. They will assume the parental responsibility for the child.
