Non-urgent advice: STEP 1: Complete and submit the New Patient Registration form
You must register the birth of your baby at your local Council, who will issue you with a Birth Certificate. Afterwards, please submit the New Patient Registration form as linked below.

The Birth Certificate is necessary to properly document the parents/guardians of the newborn. They will assume the parental responsibility for the child.
