WE ARE MOVING TO NEW PREMISES

We are delighted to announce that Chiswick Family Practice will be moving to large, modern premises in Acton in the next few months.

We will contact you again, closer to the time of the move, with an exact date for the opening at the new location, but we wanted to share details of the new facilities our patients will be able to experience at the purpose-built premises.

The building’s location offers enhanced disabled access and easy access by bus or tube.  The new premises will also provide our GP practice with additional clinical rooms, an extended wheelchair-friendly reception area, plus modern facilities with improved technology.

In comparison to our smaller, older building, these much larger premises are also being welcomed by staff. The new premises will also provide us with training facilities and our patient consultation groups will also have the opportunity to meet more easily.

We are excited about our move and will be updating patients via information in our current reception area, our website https://www.cfpdrsbhattandszyszko.nhs.uk/  and through text messages. We have also included answers to some potential queries below.

We would be grateful if all patients can ensure that their contact details (address, email and phone numbers) are up to date with the surgery so they can be kept informed of future developments.

  • What is happening?

In the next few months Chiswick Family Practice will be moving from its current location on Southfield Road to premises of the large, purpose-built facility on Acton Gardens Development. 

  • Why is Chiswick Family Practice moving?

Our current building is much older and smaller in comparison to the new facilities which will provide 551sqm of space and will benefit our patients with additional clinical rooms, enhanced procedure rooms, a wheelchair accessible reception and waiting area. Working in a more modern building with enhanced technology – with easy access to transport – is also being welcomed by staff.

 

  • When is the move taking place?

Beginning of 2022. We will write again to confirm an exact date for the move and the closure of the current practice site. Updates will also be posted on the GP practice website and in reception areas. Patients will also be contacted via text.

  • Where is the new building located?

The new location is Acton Gardens, Bollo Bridge Road, London, W3 8DA. The GP practice will occupy two floors of the building.

  • How far is the new building from the current GP practice?

Chiswick Family Practice’s new premises are 0.7 miles from the current practice. According to Google maps this is an estimated 2 min drive or 15mins walk.

We are very lucky to be able to provide medical care to our Patients and we do hope that you all will come with us to the new premises.

COVID-19 VACCINES

You can get the COVID-19 vaccine if:

  • People aged 18 and over
  • People at high risk from COVID-19 (clinically extremely vulnerable)
  • People with other conditions that put them at higher risk
  • Carers
  • People who live and work in care homes
  • Frontline health workers
  • Frontline social care workers

PLEASE NOTE THE INTERVAL BETWEEN 1st AND 2nd DOSES REMAINS AT A MINIMUM OF EIGHT WEEKS (there are some exceptions to this which will be judged on a case by case by the vaccination lead).

Couch to 5K: A Running Plan for Beginners

Couch to 5K is a running plan for absolute beginners.  The plan involves 3 runs a week, with a day of rest in between, and a different schedule for each of the 9 weeks.

 

Launch of Online Consultations

We now offer Online Consultations.   This can be accessed from our website  by clicking on the large “eConsult” banner that you can see on the Home Page. This allows patients to ask for advice about their health conditions, symptoms or request sick notes and GP letters.  eConsult is available around the clock, day or night, and on the weekend. We will get back to you by the end of the next working day at the latest.

This means you won’t need to come into the surgery unless you need a face to face appointment. You may receive a call, email or text from the surgery with next steps, or to let you know your prescription is ready to collect.

TEST RESULTS

Please note test results can be given online via the NHS App

Please note that the new NHS App is a simple and secure way to access a range of NHS services on your smartphone or tablet. To use the NHS App you must be aged 13 and over and registered with a GP surgery in England.  The app is free and easy to download from the App Store or Google Play.   • Our old online portal can still be used  (patients need to complete request form+provide  a photo ID in order to obtain log in details). The portal is accessed using this link https://systmonline.tpp-uk.com/

Altenatively test results can also be given over the phone (Mondays and Fridays between 1pm-2pm)

Why haven’t I received my test results?

We will contact you if your test result comes back abnormal and any action is required. If you don’t hear from us, it means everything is normal.

If you don’t hear from us, but would still like to know the result of your test, you can still access the NHS App or call the surgery (Monday or Friday between 1pm – 2pm) to discuss this over the phone with the Practice Nurse.

 

 

Practice Services during COVID-19 Pandemic

Please note that ALL clinicians appointments are TRIAGED by telephone first.

All screening appointments have now been stopped and nurses and HCAs are now only seeing patients for blood tests, wound care and childhood immunisation.  They are available by telephone for advice and are contacting high risk patients via telephone or video consultation.

Infection Control and Prevention:  Changes in GP Practice Operating Procedure:

  • The patient should come alone unless the patient is a child or a minor.
  • The patient should not come with extra bags, coats, etc.
  • Patients will have to wait outside the practice before being called in by practice staff.
  • Patients not to touch surfaces, door handles or use the toilets.
  • If patients are waiting in the waiting area with other patients, ensure that 2 metres distance is maintained.

If you think you may have Corona Virus PLEASE DO NOT ATTEND THE SURGERY OR GO TO THE  HOSPITAL.

Self-isolate for at least 10 days – If you live with other people, they must isolate for 10 days from when your symptoms started.
